Condition-Specific Dosing
2.5 mg
Twice daily with or without food in combination with aspirin (75 mg to 100 mg) once daily
When starting therapy after a successful LER procedure, initiate once hemostasis has been established
No dose adjustment needed based on CrCl

Renal Dosing Considerations
CAD and/or PAD: For patients with CrCl <15 mL/min, no data are available, and limited data are available for patients with a CrCl of 15 to 30 mL/min. Clinical efficacy and safety studies with XARELTO® did not enroll patients with end-stage renal disease (ESRD) on dialysis.
CrCl = creatinine clearance; LER = lower extremity revascularization.
